Omeprazole, a member of the proton pump inhibitor (PPI) class of medications, is designed to decrease the acid production in the stomach. It is effective in treating various conditions, including:

Omeprazole, a substituted benzimidazole, acts as an inhibitor of gastric acid secretion. It works by blocking the hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in gastric parietal cells. After oral administration, the antisecretory effect begins within one hour, reaches its peak effect within two hours, and can inhibit secretion for up to 72 hours. Once the medication is discontinued, gastric acid secretion gradually returns to normal over 3 to 5 days.
Omenix may reduce the absorption of ketoconazole due to decreased intragastric acidity, similar to other acid secretion inhibitors. It is metabolized in the liver via cytochrome P450, which can delay the elimination of diazepam, phenytoin, and warfarin. Monitoring of patients on warfarin or phenytoin is advised, and dosage adjustments may be necessary. However, concomitant treatment with Omenix 20 mg daily did not affect the blood concentration of phenytoin or coagulation time in patients on continuous warfarin therapy. The plasma concentrations of Omenix and clarithromycin may increase during co-administration, which is beneficial during H. pylori eradication. There is no evidence of interactions with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxicillin, or antacids. Alcohol or food does not affect the absorption of Omenix. There is no evidence of interaction with piroxicam, diclofenac, or naproxen, which is advantageous when patients need to continue these treatments. Simultaneous treatment with Omenix and digoxin may increase digoxin's bioavailability by 10% due to higher intragastric pH.
Omeprazole is contraindicated in patients with known hypersensitivity to it. Before initiating treatment with omeprazole in patients suspected of having a gastric ulcer, malignancy should be excluded, as treatment may mask symptoms and delay diagnosis.
Three prospective epidemiological studies indicate that Omeprazole does not adversely affect pregnancy or the health of the fetus or newborn. It can be used during pregnancy. However, there is limited information on whether Omeprazole passes into breast milk or its effects on neonates. Therefore, breastfeeding should be discontinued if Omeprazole is deemed essential.
Avoid using clopidogrel concurrently with Omenix, as it may reduce the effectiveness of clopidogrel. Observational studies suggest that proton pump inhibitor (PPI) therapy may be associated with an increased risk of osteoporosis-related fractures of the hip, wrist, or spine. Atrophic gastritis has occasionally been noted in gastric corpus biopsies from patients treated long-term with Omenix. The concomitant use of PPIs and methotrexate may lead to methotrexate toxicity.
